To initiate our contact or to schedule an appointment, I prefer to be emailed at email@example.com. You may use the box below, or email me in your usual fashion. In your email please include your full name, best telephone number to reach you at, and what times would be best to call you back. You may include any other personal information you would like me to know prior to our phone discussion. If you do not have access to email, please feel free to call me at 845-639-3098, keeping in mind that I may be in a clinical situation at which time I may not be able to answer the phone to an unfamiliar number. Under these circumstances please leave me a detailed voice message with your name, telephone number, a brief statement about the reason for your call, and best times to return your call.
I am known for returning calls and emails very quickly, and even if I cannot have a lengthy discussion at the time I receive your email, I very likely can respond with an acknowledgement that I have received it and that I will be returning your call as soon as possible. I do like to have enough time during our initial phone discussion without noisy interruptions to answer your questions thoroughly, explain how my practice works, and sometimes that time segment may be later in the day. You can be assured that I will be returning your call within 24-48 hours of your inquiry, and usually quicker than that.
Because I only have two spots per month for patients in my schedule, calling early in your pregnancy is better than calling later. I do accept transfer patients into my practice if you have had documented prenatal care up until that point, and I may have an open spot or be able to open a third spot depending on how far along in your pregnancy you are.
I am happy to schedule a free consultation for us to meet each other prior to the beginning of your prenatal care so we both can see if I would be the right care provider for you. Choosing your midwife is such an important decision, I welcome families to meet me, ask me lots of questions, and feel comfortable with their decision prior to making such a commitment.
Appointments are made individually, according to availability of both of our schedules, which we will coordinate by phone or at our subsequent visits. I am available on the weekends and in the evenings as well as weekdays after 12 noon.

I am an Out-of Network provider, so that those with Out-of-Network insurance benefits will be able to reasonably meet their financial responsibility for my professional midwifery services. I am not contracted, or “In-Network” with any specific insurance company. It would be helpful for those inquiring about my services to obtain prior to our discussion the following information to guide more specifically our discussion about what your out-of-pocket fees would be:
- Name of Insurance Company
- Do you have Out-of-Network Benefits?
- If so, do they cover CNMs delivering at a hospital (yes, I have a collaborative/supervising physician, however I am not employed by a physician.)
- How much is your Deductible?
- How much of your Deductible is already met for this year cycle?
- What are your Out-of-Network Benefits (i.e. they pay 80%, you pay 20%, or 70/30/ or 60/40, etc.)
- When does the “new year cycle” start (usually it is Jan-Jan, or July-July)
At the time of our initial phone discussion or free consultation, I will explain my fee schedule and payment contract.
Those who do not have Out-of-Network insurance benefits are offered the Self-Pay option with payment for my midwifery services offered at a substantial discount.
This page has a link to the forms that you would need to complete prior to our first clinical appointment, so that we can make the best use of our time together. These would include a Patient Registration Form, a Patient Health History form, and the Notice of Privacy Practice (HPPA) form. If you have records that you would like to have sent to me from a previous provider, there is also a Release of Medical Records request form that you can download and send or fax the signed copy to that provider so that I can receive and incorporate all of your relevant medical information generated by previous providers into my comprehensive care.
The Predetermination of Benefits Form Letter is an optional form template you may use to send to your specific insurance company to try to obtain the amount that they will reimburse for Out-of-Network global maternity care, so as to better be able to estimate what your out of pocket expenses would be.
